UNINSURED SERVICES

OHIP pays for most medical services however some services and procedures are considered as not medically necessary by OHIP thus uninsured. It is Parkwood Medical Centre's policy that these services are billable to the patient and payment is due upon receipt of the service.

Our fees are as follows as suggested by the Ontario Medical Association (OMA).

Visit – without Valid OHIP Card   $50
Visit – Out of the Country Initial Assessment  $100
        Regular follow-up  $50
  Follow-up with discussion of results/ re-assessment/ counselling  $100
 
TB Skin Testing (For Employment)  
   1st Step (Administration & Reading)  $40
   2nd Step (Administration & Reading)  $40
Wart Treatment  $20


Medical Forms/ Reports:

 Back to Work/ Sick Notes using Clinic Standard Template  $30
 Allied Health Referrals (Massage Therapy/Chiropractic/ Physiotherapy/ Acupuncture/ Compression Stockings (per request)  $30
 Pre-employment, Fitness, Hospital Form  $40 & up
 School/College/University Sick Note  $40 & up
 Medical Letters per request, depending on length  $30 & up
 Replacement of Immunization Card  $20
 Lost Notes or prescriptions  $15
 Copy of any results (per page)  $1
