Registration - Existing Patients

Instructions

1. Click on <Existing Patients> button if you had been at the clinic previously as a patient

2. Click <SEARCH TO START>

3.  Enter the first 5 digits of your HEALTH CARD Number

4. Select corresponding name from the Drop-down menu (ex: L***, A***)

5. Enter your Date of Birth in this format: YYYY-MM-DD

6. Click NEXT tab

7. Ensure that all information are up-to-date. Make corrections if necessary

8. Enter your REASON for Visit

9. Click DONE tab

10. You will be contacted after 9.30 am
